• 4-H Participation Form (Failure to complete this form in its entirety will result in the person being ineligible to participate in the Junior Dairy Show activities).

• Exhibitor's Entry Form (online-no mailing required)

• Exhibitors Entry Form (.pdf-print and mail-in)-(Please make a separate card for each entry.  Please make sure the writing is legible to insure the registration information is correct).
